This is super common, especially when gear has a lot of hormone in it. Basically, the hormone has crashed out of the oil. There is an easy fix. You'll want to place your vial in a zip lock bag, seal it, and dip it into a pot of boiling water. Leave it in the water, for 10-20 sec at a time, remove, shake the vial and repeat. It will take a little time, but you'll see that the crystals will get smaller and smaller. As long as you keep it in an area that is not too cold, it will probably not happen again. I would keep an eye on it and if it does happen, repeat the process. 

This probably seems like a no brainer, but be sure to let it cool back down before you inject!
