I would suggest you do not use Nolvadex with a 19-Nor as there is a chance it can make the progesterone receptor more sensitive. Further, there is no real reason for Nolvadex on cycle anyway. You'd be much better off limiting the amount of estrogen that is produced to begin with. If estrogen is under control, there is less of a chance that you will have an issue with proges or prolactin to begin with. And if you do find that you still do end up having issues with prolactin, use some caber at .5mg 2x wk to combat it.
