I'd believe that it's the tren ace causing the issue. Ace is a very short ester and needs to be injected at least EOD, if not ED to keep levels stable. When hormones are in flux, you are more likly to suffer side effects such as acne. As it is, 100mg of tren ace a wk, its really enough to do much for muscle growth. I would up the dose to 1/2 cc EOD and see how you do with that. 

Outside of that, make sure to shower at least 2x day, using dial antibacterial bar soap, use a fresh towl each time, change your sheets and pillow case at least a couple times a wk and dont wear the same shirts one more than once before cleaning it.
